The Manton Adolescent Wellness Center
(in the Manton High School) will be offering physicals
for 10-21 year old students on June 18 and June 19, 2019.
A physical, also known as a well-child visit includes: vision/hearing screening, complete physical exam, age appropriate risk assessment, immunizations, and health education. We can also clear your student for sports with the MHSAA sports physical form at this visit.
We accept all insurances; most insurances cover one physical per year. If you would like to see your primary care doctor for a well-child visit, we accept a recommended donation of $25 in place of billing your insurance. Services will not be denied if you are unable to make a donation. We can also assist you in Medicaid enrollment if desired.
